Lots of safety lessons were learned this week. The NTSB found that the decision to conduct a vent and burn of tank cars carrying vinyl chloride monomer was based on incomplete and misleading information. Trish and I discussed how corrosion caused catastrophe (and launched a 19-ton piece of shrapnel) in Philly and Jon Katz examined how the great molasses flood of 1919 is still a teachable incident.
