Looking back to move forward in terms of process safety is always a worthwhile endeavor. Trish Kerin, director of the IChemE Safety Centre, and I discussed what happened 35 years ago at the Phillips Petroleum Complex and how it led to the creation of the Mary Kay O’Connor Process Safety Center at Texas A&M.
